We understand that not one care plan fits all. Daily services can include anything from meal preparation, hygiene, cleaning, and supervision. We will take the time to get to know you and develop an individualized care plan that fits your specific needs.
We involve the family when discussing a Support Plan as it is very important that the people that know the Service User best have a say about what care is required.
The family know better than we do
We give family members access to support plans through the Family Web Portal "Birdie"
With Birdie you can see which carers are attending your loved one and what times, you can see what care has been given.
You can see the carer comments and any concerns raised.
You have complete visibility of the care call as it happens
And importantly you as a family decides which members of the family can see this information

Activities Are So Important |
---|
We work with the family to discover the activities that our Service Users enjoy. Our carers get very involved in making sure that the Service Users enjoy the time spent together For instance we have one Service User who enjoys karaoke and the carer sings along and encourages them to express themselves Another Service User helps out at a local community event every Saturday helping the homeless. Our carers go along with him to help with them Daisy and Tom are Dementia Friends Champions and run Information Sessions throughout the County. It is very important for anyone living with Dementia to get the right support as anyone can live well with Dementia. The Information Sessions are free to anyone who wants to know how to support those living with Dementia |
