Our Care team offers nursing care for people who have complex medical needs, and intermediate care as an alternative to hospital stay. Our team is here to help residents complete their recovery and prepare them to feel confident and independent when they return home.

We work in close partnership with NHS Leeds Clinical Commissioning Groups and local Health and Social Care partners to provide Intermediate Care

Our Building and Facilities

You’ll find Green Lane Intermediate Care Centre is a purpose-built facility that has a warm and friendly atmosphere. 

There is something enjoyable and stimulating for everyone to enjoy each day. Our range of activities and events at Green Lane is designed for all interests and abilities to encourage people to stay active and involved.

Everyone is encouraged to enjoy the lovely gardens and outside spaces. We also have two television lounges and a quiet sitting room. They can be used by patients for reading; watching television; quiet time or for meeting family, friends and visiting professionals.

For meal times, there are two dining rooms for communal use and we encourage patients to enjoy their meals in company. There is also a café located on the ground floor and we invite both patients and visitors to enjoy the free tea and coffee making facilities.

All 34 rooms are en-suite and have their own TV point. Wi-fi is available throughout the home. Patients can make it feel even cosier by adding personal touches such as cherished pictures, soft furnishings and mementos.

Description

We have a Centre Manager on duty during office hours and an on-call service for our senior colleagues at the weekends to ensure everyone is given plenty of support.

The care team is led by experienced and qualified Intermediate Care Nurses, with a wider Multi-Disciplinary Team including Physiotherapists and Occupational Therapists. The service is supported with enhanced GP input and other healthcare specialists as required.

The Importance Of Meal Times

Meal-times are a highlight of the day at Green Lane and a great opportunity to meet other patients and members of the care team. We encourage all patients to go to the dining room for all meals unless they unable to do so due to their condition or choice.

Our Chefs take pride in the variety of delicious menu ideas using fresh, nutritious ingredients and of course we are happy to accommodate any special dietary needs.

The daily menu is displayed in the dining room for patients and relatives to view.

Our Chef will speak with each patient either on arrival or within 24 hours to discuss special dietary needs and any likes or dislikes a patient may have.

Activities Are So Important

There is something enjoyable and stimulating for everyone to enjoy each day. Our range of activities and events at Green Lane is designed for all interests and abilities to encourage people to stay active and involved.

Another way we can help our residents to feel at home is for them to do familiar things. So our dedicated Activities Leaders gently encourage residents to carry on doing things they enjoy. If someone had a hobby or pastime they did before they arrived, we’ll encourage them to continue it with us. Or perhaps they may like to learn something new.

The important thing is that we treat every resident as an individual. If they enjoy sharing time with others, either in our home or on an organised trip out, they’re most welcome. Or, if they prefer quiet time alone, we’ll respect that too.
